Whether you are simply browsing our website to determine whether or not you would like Dr. Collins to be your General Dentist, or you have already called and scheduled an appointment, please be sure to look over this page for a few brief instructions on how we can make your visit to our office as smooth as possible.
New Patient Forms
We have provided our forms here for you to print out in PDF format. We have a copy of these forms in our office as well, so you do not have to fill them out ahead of time if you do not want to. However, it is convenient for both you, and our office staff, to come to your appointment with your paperwork already completed. That way, your waiting period while your paperwork is processed and your chart is created is significantly decreased.
New Patient Instructions
- Please fill these forms out to your best ability before arriving to our office.
- Please arrive a few minutes early. That way we will be able to make your chart quickly and keep your appointment running on schedule.
- If you do not fill out these forms ahead of time, please arrive at least 15 minutes early for paperwork.
- Please carry with you your Dental Insurance Card, and your Driver’s License or ID
- Our Patient Appointment Reminder Service is digital, so it is only a courtesy extended to existing patients who are already entered into our computer system. Please take care to keep track of your appointment with us.
Insurance & Payment Options
We provide for most PPO insurance plans, as well as imdemnity plans. The following are some of the plans that we are contracted with. However, when making an appointment please be sure to verify your insurance.

We accept all major ATM cards, Cash and Personal Checks, and Credit Cards. For our patients' convenience we have arranged a payment plan through a third party. It is our priority to see that our patients receive the treatment that they need, and that financial situations are not a bar to their health.
